Job Description:
Required Qualifications:
- Shall be a US Citizen with the ability to obtain a government
security clearance
- List all basic requirement
- License - The contractor's physician(s) assigned by the
contractor to perform the services covered by this contract shall
have a current license to practice medicine in any State,
Territory, or Commonwealth of the United States or the District of
Columbia) when services are performed onsite on VA property. -
- - Board Certification: - physician(s) shall be Board
Certified/Board Eligible by the American Board of Psychiatry and
Neurology (ABPN), and be currently certified in Basic Life Support
(BLS) by the American Heart Association. All continuing education
courses required for maintaining certification must be kept up to
date at all times. Documentation verifying current certification
shall be provided by the Contractor to the VA COR on an annual
basis for each year of task order performance. All physicians
performing on this task order shall keep current the level of
certification at time of award. Copy of Board Certification from
the American Board of Psychiatry
- letter from the American Board of Psychiatry and Neurology
stating eligibility; orCopies of All Successful Completion
certificates from a US Accredited Psychiatry residency
program.
- Copy of Medical Degree shall be provided upon submission or a
current search conducted no earlier than 5 days of quote submission
from a physician profile search engine confirming the medical
degree. - - .
- Technical Proficiency: Physician(s) shall be technically
proficient in the skills necessary to fulfill the government's
requirements, including the ability to speak, understand, read and
write English fluently. Contractor shall make available documents
that verify the physicians current and ongoing competency, skills,
certification and/or licensure related to the provision of care,
treatment and/or services performed for each physician. Contractor
shall provide verifiable evidence of all educational and training
experiences including any gaps in educational history for all
physician(s) and all physician(s) shall be responsible for abiding
by the Facility's Medical Staff By-Laws, rules, and regulations
(referenced herein) that govern medical staff behavior.
- Continuing Medical Education (CME)/ Certified Education Unit
(CEU) Requirements: Contractor shall provide copies of physician(s)
current CMEs as required or requested by the VA. Physician(s)
registered or certified by national/medical associations shall
continue to meet the minimum standards for CME to remain current.
Contractor shall report all physician(s) CME hours to the
credentials office for tracking. These documents are required for
both privileging and re- privileging. Failure to provide shall
result in loss of privileges for the physician(s).
- Training (BLS, CPRS and VA MANDATORY): Physician shall meet all
VA educational requirements and mandatory course requirements
defined herein; all training must be completed by the physician(s)
as required by the VA. All employees who will be working on this
task order and require access to VA information and VA information
systems are required to complete the following training before
being granted access to VA information and its systems:
- This training is offered through the VA Talent Management
System (TMS). Each employee will be required to self-enroll for a
profile on the VA TMS site by visiting
https://www.tms.va.gov/plateau/user/login.jsp. Once there, they
should create a profile, launch the mandatory training, and
complete the content prior to starting work at the VA. Upon
completion, the Contractor shall provide the CO and COR with the
employee's printed certificates of completion from the TMS. These
certificates display the employee's TMS User ID. Failure to
complete the mandatory training annually, within the timeframe
required, is grounds for suspension or termination of all physical
or electronic access privileges and removal from work on the task
order until the training and documents are complete.
- TUBERCULOSIS TESTING: Contractor shall provide proof of a
negative reaction to PPD testing for all physician(s). A negative
chest radiographic report for active tuberculosis shall be provided
in cases of positive PPD results. The PPD test shall be repeated
annually.
- RUBELLA TESTING: Contractor shall provide proof of immunization
for all physician(s) for measles, mumps, rubella or a rubella titer
of 1.8 or greater. If the titer is less than 1.8, a rubella
immunization shall be administered with follow-up documentation to
the COR.
- OSHA REGULATION CONCERNING OCCUPATIONAL EXPOSURE TO BLOODBORNE
PATHOGENS: Contractor shall provide generic self-study training for
all physician(s); provide their own Hepatitis B vaccination series
at no cost to the VA if they elect to receive it; maintain an
exposure determination and control plan; maintain required records;
and ensure that proper follow-up evaluation is provided following
an exposure incident. The VAMC shall notify the Contractor of any
significant communicable disease exposures as appropriate.
Contractor shall adhere to current CDC/HICPAC Guideline for
Infection Control in health care personnel (as published in
American Journal for Infection Control- AJIC 1998; 26:289-354
http://www.cdc.gov/hicpac/pdf/InfectControl98.pdf) for disease
control. - -
Scheduling Details:
- List staffing/shift information
- Monday through Friday, 8:00 a.m. to 4:30 p.m.
Duties include, but are not limited to the following:
- List job duties
- 4.5.2.8. Scope of Care: Contractor's physician(s) (as
appropriate and within scope of practice/privileging) shall be
responsible for providing psychiatry care, including, but not
limited to:
- Mental Health Services: Contractor's physician(s) shall
provide
- comprehensive clinical Mental Health Psychiatry services in a
manner that is compliant with InterQual admission and discharge
criteria. 4.5.2.10. Post-Discharge Follow-Up: Contractor's
physician(s) will follow up with Veterans within for inclusion in
departmental reports. 4.5.2.11. Consults: Contractor's physician(s)
shall provide consultative services at the patient's bedside if the
patient is not ambulatory and in the clinic setting if the patient
is able to report to the outpatient clinic. Procedures shall be
scheduled for completion within 30 days of the date of the consult.
4.5.2.12. Medications: Contractor's physician(s) shall follow all
established medication policies and procedures. No sample
medications shall be provided to patients. 4.5.2.13. Discharge
education: Contractor's physician(s) shall provide discharge
education and follow up instructions that are coordinated with the
next care setting for all mental health clinical. 4.5.2.14. Mental
Health operations are to be performed in collaboration with other
mental health providers and trainees. 4.5.3. ADMINISTRATIVE:
estimated_5_% of time not involved in direct patient care 4.5.3.1.
Quality Improvement Meetings: The contractor's physician(s) shall
participate in continuous quality improvement activities and
meetings with committee participation as required by the VAMC Chief
of Service, Chief of Staff, or designee. 4.5.3.2. Staff Meetings:
The contractor's physician(s) shall attend staff meetings as
required by the VAMC Chief of Service, Chief of Staff, or designee.
contractor to communicate with COR on this requirement and report
any conflicts that may interfere with compliance with this
requirement. 4.5.3.3. QA/QI documentation: The contractor's
physician(s) shall complete the appropriate QM/PI documentation
pertaining to all procedures, complications and outcome of
examinations. 4.5.3.4. Patient Safety Compliance and Reporting:
Contractor's physician(s) shall follow all established patient
safety and infection control standards of care. Contractor's
physician(s) shall make every effort to prevent medication errors,
falls, and patient injury caused by acts of commission or omission
in the delivery of care. All events related to patient injury,
medication errors, and other breeches of patient safety shall be
reported to the COR VA Safety Policy. As soon as practicable (but
within 24 hours) contractors shall notify COR of incident and
submit to the COR the Patient Safety Report, following up with COR
as required or requested.
- Outpatient Provider Duties: Provide psychiatry services at the
WNY VAMC's Outpatient Clinic. Shifts will be Monday through Friday
from 8:00 a.m. to 4:30 p.m. except for Federal Holidays. Physician
will provide treatment/evaluation/consultations to approximately
10-12 scheduled patients per day in the regular outpatient clinic;
60 minutes for new patients and 30 minutes for follow up
patients.
- Inpatient Provider Duties: Psychiatrist will provide
diagnostic/evaluation to approximately 7-8 patients in an acute
psychiatric inpatient setting. Certification in ECT is preferred
for the delivery of ECT as needed.
- Physicians will be responsible for completing all documentation
and closes all patient encounters in a timely manner, by the end of
shift, utilizing the Computerized Patient Record System (CPRS)
- Physicians will be responsible for diagnosing, treating and
managing psychiatric patients according to current standards of
care. Services to be performed include, but are not limited to:
performing assessments/examinations on initial encounters with
unscheduled, emergent, urgent, and new patients; coordinating
patient treatment plans; ordering appropriate diagnostic tests;
recommending patients for hospital admission; and documenting in
the computerized medical record. Standards of Care: Physician(s)
services shall cover the range of psychiatric services as would be
provided in a state-of-the-art health care treatment facility and
the standard of care shall be of a quality, meeting or exceeding
currently recognized national standards as established by:
